Patient Eligibility and
Required Documentation

Patients who receive care MUST:

• Be employed with incomes within 200% of the annually published Federal Poverty Guidelines
• Be over 18 years of age*
• Not have medical insurance
• Not be eligible for Medicare, Medical Assistance, or any other insurance program
• Live in either Bucks or Montgomery County

Patient eligibility is determined before the first medical or dental visit. To determine eligibility, prospective patients must provide appropriate financial and other documentation during a scheduled eligibility determination appointment:

• Proof of identity
• Proof of work status and income (recent pay stubs, a recent W2 form, a most recent tax return, and/or an employer’s verification of wages)
• Proof of address verification

Financial eligibility is reassessed at least annually for patients who qualify for HealthLink services.

*In an effort to maximize services to the community, HealthLink focuses on adult care. St. Mary’s Medical Center’s Children’s Health Center in nearby Bensalem, Pennsylvania provides primary healthcare services for children newborn to age 18.
