Patients who receive care MUST:
- Be employed with incomes within
200% of the annually published
Federal Poverty
Guidelines
- Be over 18 years of age*
- Not have any form of medical insurance
- Not be eligible for Medicare,
Medical Assistance, or any other insurance program
- Live in either Bucks or
Montgomery County
Patient eligibility is determined
before the first medical or dental visit. To determine
eligibility, prospective patients must provide
appropriate financial and other documentation
during a scheduled eligibility determination appointment:
- Valid Photo ID
- Proof of work status and income (2 months
of recent pay
stubs, a recent W2 form,
and/or an employer’s verification of wages)
- Proof of address verification
- Most recent tax return
Financial eligibility is
reassessed annually for patients who qualify for
HealthLink services.
Additional documentation may be
required for medication acquisition.
*In an effort to maximize services
to the community, HealthLink focuses on adult
care.
